Instructions for Completing Sports Medical Forms

Medical Sport Form Instructions

2019 FALL Sports -7th and 8th grades
Girls Soccer and Boys Soccer Medical forms are due by the first day of school, September 3, 2019 by 3pm.

Personally hand all forms to Mrs. Yadoff and no one else during the school year.
Carefully read and follow the instructions below to ensure your child will have the forms approved in time for tryouts.
 
FORMS WILL BE RETURNED TO YOUR CHILD IF THEY ARE INCOMPLETE. CHECK WITH YOUR CHILD TO BE SURE YOUR FORM WAS ACCEPTED.
 
Attention all Athletesyou must drop off in person your completed sports form to the nurse by 3pm or earlier during the school year.
 
Attention all Parentsplease review all forms to make sure all sections are completed (including vision)If forms are not complete they will be returned delaying the approval process.
 
EVERY SPORT PHYSICAL IS SUBMITTED TO OUR DISTRICT DOCTOR FOR REVIEW AND APPROVAL. IN ORDER TO DO THIS BEFORE TRYOUTS YOUR FORMS MUST BE IN BY DEADLINE DATE. FAILURE TO COMPLY MAY RESULT IN YOUR CHILD NOT PARTICIPATING.

Please complete the forms below for your child/children to be eligible to tryout for EMS sports. Click the form titles below or download the forms from the Nurse's webpage.
 
1. Sports Consent Form- 1(one) consent form for the current announced sport only- Circle one sport on the form.

2. PreParticipation Forms
- (4 pages) - page 1 & 2 parent completes; page 3 & 4 doctor completes. 
IMPORTANT: THE DATE ON THE PHYSICAL EXAM SUBMITTED MUST BE THE DATE OF THE LAST PHYSICAL EXAM- (please check the doctor writes this date on the form- not the date they are completing the form). The actual physical exam submitted must have the same date as the one you wrote as last the physical exam date. 
Pre-Participation Physical Forms are valid for 365 days before the tryout date.
If your physical expires (over 365 days), before the first day of tryouts, you must submit a new updated physical exam to the school nurse.

EpiPen Information
If your child has a life-threatening allergy that requires Epinephrine he/she is not eligible to participate in sports for the current school year until the Epinephrine Packet is completed. EpiPen Packet
Asthma Action Plan Information
If your child has Asthma he/she is not eligible to participate in sports for the current school year until a signed Asthma Action Plan has been received from your private physician. If your child does not require the use of an inhaler, a physician statement indicating that an inhaler is no longer necessary must be submitted. Asthma Action Plan

3. Health History Update Questionnaire
-
  If you have a valid Pre-Participation Physical Form on file in nurse’s office (valid for 365 days) you need to only complete and submit the Health History Update Questionnaire.  (This form is only 1 page and filled out by the parent/guardian.)
Be sure the month- date- and year of the last physical is on the Health History Update Questionnaire ! This date needs to be the same date as the physical exam previously submitted.
 **This form must be completed and dated within 90 days of the start of every sports
tryout.
 
In Addition, the forms below must also be completed before tryouts. These forms  only need to be completed once for the school year.
